End-of-Grant Year Report Form

The Woods Fund views this end-of-grant year report as our opportunity to have a "conversation" with you - albeit a one-sided conversation where we ask the questions and you provide the answers - about your organization's recent experiences.

There is much to be learned both from successes and from assumptions, methods, and initatives that turn out to be not fully correct or successful. We would like you to give us as honest and complete answers as possible. We know that completing this form takes a considerable effort as it requires reference back to your Woods Fund application and a careful review of what has transpired during the past year.
